The diagnostic assessment is a tool used by licensed clinicians of 180 Community Wellness Centers to help determine the clinical needs of individuals seeking services. The assessment is conducted in an interview format and the information gathered is used to develop a treatment plan through a variety of services to assist individuals with regaining the ability to function optimally.
Think of the assessment as your roadmap. Before we can recommend the right combination of services — whether that's talk therapy, Basic Skills Training, Psychosocial Rehabilitation, or another approach — we need to understand your unique situation, history, and goals.
Assessments are available in-person at our North Las Vegas office for children, adolescents, adults, couples, and families.

Clinical Interview
An in-depth, structured conversation covering your current concerns, symptoms, daily functioning, and what you hope to achieve through services.
Personal & Family History
A review of your background including medical history, family dynamics, previous treatment, and significant life events that provide clinical context.
Treatment Plan Development
Based on the assessment findings, your clinician creates a personalized treatment plan outlining recommended services, goals, and a path forward.
